Do not dig deeply into the nail corner at home if sensation is reduced.

Thick, yellow, brittle, or crumbly nails may be associated with fungal involvement or chronic nail changes. A proper diagnosis should come from a qualified provider. The main concern for neuropathy patients is that thickened nails can increase pressure and make safe trimming harder.
